Separate myogenic, arthrogenic and disc-related TMD. Establish which of the three presentations you are treating before you choose a technique, so masticatory muscle pain, joint surface pain and internal derangement each get the management they actually need.
Run the TMJ examination. Measure jaw excursion, characterise joint sounds — the clicking and popping patients report long before they report pain — and palpate the masticatory muscles, including the lateral pterygoid, as a repeatable, chartable assessment.
Treat the jaw and the upper cervical spine together. Apply evidence-informed manual therapy and mobilization to the temporomandibular joint and the upper cervical segments as one functional unit, which is where the TMD–cervical connection is either resolved or missed.
Document TMD so it survives review. Build treatment plans with quantifiable clinical goals and navigate the specific reporting and coding requirements for temporomandibular disorders, so the file is audit-ready.
