The upper extremity is where conservative care most often runs out of road, and where the referral decision gets made badly. Evidence-Based Practice 206 works through the shoulder, elbow and wrist as a treatment hierarchy rather than a list of diagnoses: what conservative management can reasonably be expected to achieve, at what point injection or surgical consultation becomes the better option, and how that judgment is communicated to the patient and to the physician receiving the referral.
It also covers the half of collaborative care that is usually left out — what happens after. Post-injection and post-surgical management, functional restoration toward a defined activity goal, and the behavioral-health factors that predict which upper-extremity cases stall. The aim is a clinician who can hold a case through the whole arc and hand it off cleanly when handing it off is the right call.
