Low back pain is the complaint a chiropractor sees most, and the examination decides everything that follows: the source, the level, the severity, what to rule out and what the treatment plan will be. Physical Examination 208 is the two-hour course on the examination of the lumbar spine and sacroiliac joints, with the key tests demonstrated on video. Dr. Donald Ozello opens with the evidence — a 2023 comparison in which chiropractic spinal manipulation outperformed conservative therapy for low back pain on pain, range of motion, disability and general health, and the 2020 randomized trial in which active-duty military personnel receiving chiropractic care improved in strength, endurance, pain and disability against a wait-list control — then the anatomy the exam depends on: the lumbar spine and its five pairs of mixed spinal nerves, the dermatomes and myotomes, and the sacroiliac joints with the strong ligaments and muscles that bind them.
The examination follows in sequence: consultation and history; the purpose of the physical examination; palpation of the lumbar, gluteal and lower extremity muscles and the bilateral radicular evaluation every exam should include; the deep tendon reflexes (L4 patellar, L5 medial hamstring, S1 Achilles) graded 0–5; muscle strength grades; the myotomes L2 through S2 and the dermatomes L1 through S5; range of motion grading; visual inspection for antalgic gait and guarded movement; and palpation for tenderness, hypertonicity, trigger points and reproduction of the chief complaint. Then the orthopedic tests — Bechterew's, the straight leg raise and Braggard's for nerve root impingement; Ely's, Yeoman's, Hibb's, Gillis' and iliac compression for sacroiliac joint sprain; FABERE (Patrick's) to separate hip from sacroiliac pain; Kemp's for facet syndrome versus nerve impingement; the Stoop test for central stenosis; and the one-leg hyperextension (Stork) test for a pars defect — each with position, procedure, positive finding and indication. It closes with x-ray and MRI. It is the examination companion to Dr. Ozello's Adjusting & Manual Therapy 209: Lumbar & Pelvic Techniques, and the procedural counterpart to Dr. O'Hara's Physical Examination 203, which covers the lumbar spine, hip and hamstring from the injury-pattern and imaging side.

What You'll Learn
On completing Physical Examination 208 you will be able to:
- Summarize the course's evidence for chiropractic care in low back pain and describe the anatomy of the lumbar spine, its five pairs of mixed spinal nerves, the dermatomes and myotomes, and the sacroiliac joints
- Conduct the lumbar consultation and visual inspection, palpate the lumbar, gluteal and lower extremity muscles for tenderness, hypertonicity, trigger points and reproduction of the chief complaint, and perform the bilateral radicular evaluation
- Perform and grade the lower extremity neurological screen — deep tendon reflexes at L4, L5 and S1 on the 0–5 scale, muscle strength testing, the myotomes L2–S2 and the dermatomes L1–S5 — and grade range of motion
- Perform and interpret the nerve root tests (Bechterew's, straight leg raise, Braggard's), the sacroiliac tests (Ely's, Yeoman's, Hibb's, Gillis', iliac compression with its contraindications, FABERE/Patrick's), Kemp's test for facet syndrome versus nerve impingement, the Stoop test for central stenosis and the one-leg hyperextension (Stork) test for a pars defect
- Select lumbar x-ray or MRI by indication — disc space and foraminal narrowing, fracture, dislocation, pathology and space-occupying lesion, disc herniation and nerve root compression, malignancy and inflammatory conditions

Frequently Asked Questions
What does Physical Examination 208 cover?
The examination of the lumbar spine and sacroiliac joints as a chiropractor performs and documents it: the evidence for chiropractic care in low back pain; the anatomy of the lumbar spine, its spinal nerves, dermatomes and myotomes, and the sacroiliac joints; consultation and history; palpation and the bilateral radicular evaluation; the lower extremity neurological screen — deep tendon reflexes at L4, L5 and S1 graded 0–5, muscle strength grades, myotomes L2–S2 and dermatomes L1–S5; range of motion grading; visual inspection and palpation findings; the orthopedic tests — Bechterew's, straight leg raise, Braggard's, Ely's, Yeoman's, Hibb's, Gillis', iliac compression, FABERE (Patrick's), Kemp's, the Stoop test and the one-leg hyperextension (Stork) test — each with position, procedure, positive finding and indication; and x-ray and MRI. The key tests are demonstrated on video.
Who teaches the course, and who is it for?
Donald Ozello, DC, CCN, CCSM — sports chiropractor, founder of Championship Chiropractic in Las Vegas and CCEDseminars faculty for the Sports Injuries, Rehabilitation and Adjusting & Manual Therapy series — teaches the 2-hour course. It is written for Doctors of Chiropractic who want a repeatable, documentable low back examination: the order of the sequence, the neurological grades that belong in the chart, and each orthopedic test with what a positive result means and which structure it points to.
How does a chiropractor tell whether low back pain is coming from the sacroiliac joint, the facet joints or a pinched nerve?
By letting the tests sort it, which is what the course teaches test by test. A nerve root under pressure in the lumbar spine declares itself on the straight leg raise, Braggard's and Bechterew's tests — sharp pain, numbness or burning down the leg — and on the neurological screen, where a weak myotome, a numb dermatome and a diminished reflex point to the level. Kemp's test separates the facet joint from the nerve: the patient is taken through flexion, lateral flexion and extension, and pain that stays in the low back on the tested side means facet syndrome while symptoms that radiate into the leg mean nerve impingement. The sacroiliac joint has its own set — Yeoman's for the anterior ligaments, Hibb's for the posterior ligaments, Gillis', Ely's and iliac compression — and FABERE (Patrick's) settles whether pain on the hip-opening maneuver is the hip itself or the sacroiliac joint. The Stoop test picks up the central stenosis patient who leans forward to walk, and the one-leg hyperextension test points to a pars defect. The pattern across the tests, not any one of them, makes the call.

What pairs well with Physical Examination 208?
Dr. Ozello's Adjusting & Manual Therapy 209: Lumbar & Pelvic Techniques is the treatment half of the same region — 208 is the examination, that course is the adjusting. His Physical Examination 205: Cervical Spine and 207: Thoracic Spine and Rib Cage complete the spinal examination from the occiput to the sacrum on video; the three together are 6 CE hours, where the automatic 10% multi-course discount starts ($108 instead of $120). Dr. O'Hara's Physical Examination 203: Lumbar Spine and Lower Extremity approaches the same region from the injury-pattern and imaging side — spondylolysis staging, the lumbar disc, the hip and hamstring — and many clinicians take both. New Physical Examination titles are released as faculty complete them; the current list with pricing lives on the category page.
State approval and licensing reminder. CCEDseminars is PACE Provider #34015544. Approved states for this course are listed at the bottom of this page, and board rules on online hours change — confirming your own board's current requirements before you enroll is your responsibility.
